An antioxidant is a nutrient that is in charge of disposing, scavening and halting the production of ROS or neutralizing their actions. The main antioxidants are vitamin A, vitamin E (tocopherol or tocotrienols), vitamin C, and trace minerals. Nutrients in food supplements such as selenium, zinc, carnitine, arginine and vitamin B-12 have been shown to increase sperm count and motility. Further, antioxidants such as vitamin C, Coenzyme Q, vitamin E and glutathione have been reported to be effective against male infertility (1). Several studies have concluded that the combination of these nutrients double the motility and the number of sperms in only three months (2).

Vitamin A: The well known vitamin A is a group of unsaturated nutritional organic compounds that includes retinol, retinal, retinoic acid and various provitamin A carotenoids such as beta carotene(3,4). Vitamin A has important antioxidant properties, and is needed by men to maintain healthy sperm cell membranes. It regulates genes that control sperm production (spermatogenesis). A lack of vitamin A may lower sperm count (5-7). Aditionally, it is the most important vitamin for development and growth, whose benefits are externally revealed through an improvement in skin conditions such as psoriasis (red, itchy and scaly skin) and acne. Aditionally, vitamin A has a direct effect in the Thymus gland, which is the main gland of the immune system.

Vitamin C is a six-carbon keto-lactone which is present in the human body and it is biosynthesized in the liver. The body can not keep it for long periods of time therefore it is necessary to eat food rich in vitamin C or take it in a supplement. Vitamin C helps in the metabolic processes of folic acid, tyrosine and tryptopan (8), it is also well known for its role in tissue growth and wound healing (9,10), and it is very powerful in scavenging ROS (11,12). Vitamin C is highly present in the ejaculated fluid. A lack of vitamin C (generally often seen in smokers) can lead to infertility and damaged sperm cells. A study with 75 smoker men taking daily 200mg of vitamin C during 60 days revealed that the number of healthy sperm reached around 112% and 140% in participants taking 1000mg. Besides, every single man in the study was able to conceive successful (13). Aditionally, Vitamin C protects spermatozoa against oxidative damage by eliminating the effect of radicals (hydroxyl, superoxide and H2O2). Semen samples with excess ROS are correlated with very low concentration of vitamin C (14). Due to the fact that vitamin C works closely with zinc in its metabolism it is recommended to take it together with zinc. Further, the combined intake of vitamin C and E has been found to protect the sperms against peroxidative attack and DNA fragmentation (15).

Vitamin E: Is a strong fat-soluble antioxidant located in the cell membrane. It has the crucial function of protecting the membranes of the cells. Several studies on male fertility have shown that vitamin E can considerably improve the sperm motility (16) and the sperm cells’ ability to penetrate the egg cell (17). A study conducted by Suleiman et al. in 1996 demonstrated that vitamin E Supplements can significantly reduce lipid peroxidation in seminal plasma, improve sperm motility and increase the pregnancy rates (18,19). In a further study was concluded that the intake of vitamin E and selenium during six months resulted in an significant increase in sperm motility and a reduced percentage of defective spermatozoa (20,21).

Vitamin B12 actively works in sperm production and is important for DNA maturation and duplication. A lack of vitamin B12 has been associated to erectile dysfunction and a decrease in sperm count and infertility may be the consequence. Several studies have demonstrated that B12 supplements can help sperm count (22,23). Vitamin B12 is also important for metabolism and red cell production, which is associated to an improved circulation and thus to a healthy erectile function.

Coenzyme Q10: Is a fat-soluble substance similar to a vitamin and works as an antioxidant fighting the effects of free radicals. Coenzyme Q10 is found in every cell of the body and in the centre of sperm cells. It is crucial for the body's and sperm's energy production. Several studies have concluded that the intake of Coenzyme Q10 has positive effects in the motility and count of sperm cells. A further study concluded that Coenzyme Q10 increased sperm density, motility, and morphology.(24)

Selenium is a trace mineral that has anti-oxidant properties, it is required for sperm maturation and protects the cell membrane of the spermatic cells from oxidization (prevents lipid peroxidation), which is very important due to the fact that sperm have a very delicate fatty acid composition (25-27). Selenium is part of the antioxidative enzymes, which influence the sperm quality regarding life, motility and form. Further, it is an activating component of the enzyme glutathione peroxidase, which protects your cells from free radical damage. Selenium works synergistically with other vitamins like vitamin E and Vitamin A. Additionally, Selenium contributes to a reduction in difficulties with processing oxygen.

Zinc is an an essential trade mineral that plays a role in sperm formation, testosterone metabolism, and cell motility. Besides, zinc is very important in the formation of more than 200 enzymes and is responsible for a normal reproductive function in men. A lack of zinc leads to male sexual hormone (testosterone) deficiency and thus to a lower number of sperm cells. Several studies demonstrated that zinc intake considerably improved the average sperm count and motility in less than two months and increased the pregnancy rate in four months or less (28). A minimum of 15 mg. daily zinc is needed by men.

Arginine: Arginine is a semi-essential amino acid. Due to its nitrogen content, it improves blood circulation even in the smallest type of blood vessels (also called tissues with thin capillaries). L-arginine takes part in the production of Nitric Oxide (NO), which is vital in regulating the tension of blood vessels. By doing this, NO regulates the rate of blood flow and with this, it regulates the supply of nutrients along the whole body. The regulation of blood vessels is crucial for the healthy functioning of arteries and blood circulation. This is the reason why urologist recommend arginine as a treatment against erectile dysfunction. A further contribution of arginine to male fertility is the fact that high amounts of this semi-essential aminoacid is needed for the creation of sperm and other chemicals in semen. Having a lack of arginine has been observed in connection with having a lower sperm production. In the same way, it has been observed that arginine supplements foster a better sperm development in terms of more ejaculate and better motility (29). Further, if 300mg of arginine is taken daily, it increases the probability of getting pregnant because it strengthens the erection and thus a more frequent intercourse.

Carnitine: Carnitine is a substance similar to amino acids, which is in charge of transporting fatty acids into the centre of the cells so that energy is provided. Low carnitine levels have been observed in relation with obesity and metabolic problems with weight. Carnitine in high amounts is present in the male epididymis, which is an organ located at the back of the testicles and is responsible for sperm production. Several studies with smokers and non smokers men have demonstrated that the intake of carnitine supplements specially by smokers, increases the quality of sperms in terms of the number of sperm cells, their movement and the number of sperms with a normal shape also called morphology.(30)

L-cysteine: This nutrient is classified as a “semi-essential” amino acid because it can be made in small amounts by the human body. Together with two other amino acids, glutamine and glycine, L-cysteine is needed to make glutathione, which is the most porwerful antioxidant in the human body. 

L-cysteine supplementation, also known as N-acetyl-L-cysteine (NAC), is important for its ability to increase glutathione levels in the body, which is extremely important for lung function, brain function and liver detoxification.

Regarding male fertility, supplementation with L-cysteine in the form of NAC has been demonstra-ted to decrease the concentration of damaging reactive oxygen species within semen, which increases sperm motility. The effects of NAC supplementation were assessed during three months on 120 men diagnosed with idiopathic infertility (31). The results were a significant improvement in semen volume, viscosity, and sperm motility. Besides, an improvement in the total antioxidant capacity was shown as well as a decline in the reduction in destructive reactive oxygen species. Therefore it is concluded that the ability of NAC supplementation to improve sperm count and motility increases the potential for fertilisation. In the same way, an enhancement in the semen viscosity is also important because this makes it easier for the sperm to swim quickly forward towards the egg. In another study was found that NAC together with the trace element selenium is beneficial for male reproductive health. The study took 26 weeks and analysed the effects of selenium and NAC on 468 infertile men diagnosed with idiopathic oligo-asthenoteratospermia (26).

Subjects were randomly assigned to either a placebo, 200 microg selenium orally daily, 600 mg N-acetyl-cysteine orally daily, or 600 mg N-acetyl-cysteine plus 200 microg selenium daily over a period 26 weeks. The results showed that all semen parameters were significantly enhanced following the combination therapy. Sperm concentration increased, sperm motility improved, and the percentage of sperm with normal morphology also increased.
